Boarding & Daycare Form

I agree that to my knowledge, my dog is obedient and non-aggressive.

 

I am aware that my dog(s) will be released into the park, off-leash, with other dogs.

 

I understand that my dog must be up to date on all of their vaccines and that my dog must be on flea prevention and parasite-free. If my dog is flea- or parasite-infested, I give permission to treat if necessary.
